Home Modification Report

A home modification report from an expert will outline what needs be done to the injured party's home to make it accessible for them. The extensive report will include detailed analysis of each area of the home and necessary drawings of existing and proposed floor plans to supplement the detailed scope of work provided.

Home Modification Assessment and report may include (needs are project specific):

Review of medical history
Assessment of person, function, and equipment
Extensive assessment of home environment
Follow up report including a scope of work for bidding
Specification of products, DME, and materials matching specific needs
Schematic Auto-CAD floor plans of existing and proposed layout(s)
Travel within Greater Portland area. Other areas can be serviced virtually or for an additional travel fee.
Services available throughout the United States.
Available to present project virtually to carrier or involved parties - additional charges may apply
Available for litigation - additional charges will apply

Priced per project, usually ranging between $1500-$3000

Needs / Life Care Assessments

We understand how to assess your client's current and future needs to generate a detailed Needs / Life Care Assessment report. As a licensed occupational therapist, I am uniquely positioned to evaluate functional skills (ADLs and IADLs) skilled needs, and to extrapolate their needs over a lifetime.

Life Care Assessment and report may include (needs are project specific):

Review of medical history
Detailed assessment of body systems
Detailed assessment of functional abilities
Detailed assessment of equipment (DME)
Detailed assessment of necessary medical supplies
Detailed assessment of use of skilled and non-skilled services
Extensive assessment of home environment
Report listing medication schedule
Travel within Greater Portland area. Other areas can be serviced virtually or for an additional travel fee.
